If this is your first visit to Southeastern Rehabilitation Medicine, welcome!

Appointments are necessary to be seen at our clinic. Same or next day appointments are available for urgent concerns. When you arrive we will need to verify your insurance and will collect your identification as well. There will also be some forms for you to fill out while you wait so that we can find out about your medical history, current and past medications and the reason for your visit.
