Appointments are necessary to be seen at our clinic. When you arrive, we will need to verify your insurance and will collect your identification as well. There will also be some forms for you to fill out while you wait so that we can find out about your medical history, current and past medications and the reason for your visit. If you would like to expedite your visit, you can download some of these forms by clicking here and fill them out before you arrive. We recommend that if you are a first time patient, to please arrive 15 minutes before your appointment.

Start out going south on US-72/US-441 south toward Morse Blvd. Turn right onto CR-466/Hwy 466 and continue to follow. Turn left onto Morse Blvd. and then pass through one roundabout. At the next roundabout, take the first exit onto Lake Sumter Landing. Turn left onto Old Camp Road. 910 Old Camp Road #202 is on the left.
